BALANCE Workshop Volunteer Filming Participation Form

Thank you for volunteering to help with the filming of our program! Your help is very much appreciated. Please remember to bring two extra shirts to change for the scenes. A pizza lunch will be provided for the children at the end of filming the segments.

Parents/Guardians, we need you too! If you would be brave enough to come at 12:15, we would like to film a parent session. You are welcome to contribute verbally or just be a supporting body in the mix!

By creating these videos, we will be able to offer training to facilitators to run this program almost anywhere in the world!

Waiver and Release Information

By submitting the enrollment form, you agree to the following:

BALANCE Workshops AGREEMENT of RELEASE and WAIVER of LIABILITY for BALANCE Workshops I agree with the following on behalf of my family members and myself: 1. My child(ren) and I are participating in the yoga workshop sessions during which we will receive instruction. I recognize that yoga requires physical exertion, which may be strenuous. I am fully aware of the potential risks. 2. I understand that it is my responsibility to consult with a physician before and during the yoga workshop sessions. I represent and warrant that my child(ren) and I are physically fit and have no medical condition preventing our full participation in the yoga workshop sessions. 3. I will assume full responsibility for any risks, injuries, or damages, known or unknown, which might be incurred due to my child(ren) and my participation in the yoga workshop sessions. 4. I knowingly, voluntarily, and expressly waive any claim my family or I may have against the instructors, assistants, BALANCE4Kids®, LLC/BALANCE Workshops, or other facilities, for injury or damages that my child(ren) or I may sustain as a result of participating in the yoga workshop sessions. 5. I hereby permit images of my family, captured during yoga workshop sessions through video, photo, and digital camera, to be used solely for the facilitator/facility and BALANCE4Kids®, LLC/BALANCE Workshops promotional material and publications, and waive any rights of compensation or ownership thereto. I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions as stated above. All facilitators are independent contractors. BALANCE4Kids®, LLC/BALANCE Workshops does not assume any liability for classes taught by instructors.
